Insurance claim printing is an important part of most dental practices. Diamond Dental Software prints the latest ADA Insurance Claim form in both a pre-printed form format, and a plain paper version that does prints on blank computer paper. Older ADA forms going back to the 1994 version can also be printed. Special forms such as California DentiCal can also be printed. The program also prints the medical HCFA-1500 form. The program also generates electronic claims. See the section on Electronic Claims for a complete discussion on this subject. Nearly important as the printing of the claim, is the tracking of the claim. It is necessary for an organized dental office to know the fate of the claims that are sent for payment and authorization. To print an insurance claim, click on the Print Insurance button from the Main Menu.  This will open the Patient Search screen.  Perform the search in the usual way.  In the lower left corner of the Patient Search screen you will notice a button entitled Batch Printing.  Batch printing of multiple insurance claims will be discussed later in this section.  After the search has been performed the Insurance Form Printing screen - page 1 will open. The Insurance Form Printing screen displays as four pages. The control buttons displayed along the bottom of the screen stays visible as you flip through the various pages of the screen. To change pages click on the Up Arrow button or the Down Arrow button.  The page number is also seen in the upper right corner of the screen. The four pages of the screen will now be discussed.

Page 1 Screen Information
Insurance Form Printing - page 1 is the opening screen for insurance printing. The items on page 1 will now be discussed.

Patient Name and Account Number
The name of the patient who was searched for during the Patient Search will be displayed on the screen along with his or her Account Number.

Payment/Authorization Option
The Payment/Authorization option box defaults to the "Payment" choice as the Insurance Form Printing screen opens. You may click on the Authorization choice if the screen is to be sent for Authorization. This choice will cause the appropriate box to be checked on the insurance claim form. When sending for Payment, procedures posted as Proposed will not automatically qualify for printing. When the Authorization choice is selected, Proposed procedures will be qualified for printing. Procedure qualifications for printing is discussed in more detail later in this section.

View How Far Back
The number entered into the View How Far Back text box indicates how many months back to display procedures for this patient. As the screen opens, the default for this field is 36. Using 36 as an example, the procedures displayed on the screen will be up to 36 months old.  Procedures posted more than 36 months ago will not display. You may change this field to any whole number you want with 1 being the smallest allowable.
